28 Bed Safe Haven in Manhattan



Full-time position 37.5 hours per week

Tuesday-Saturday 8:00am-4:30pm



Provide comprehensive case management services to a challenging caseload of severely and persistently mentally ill, homeless adults. Includes assistance with obtaining benefits, referrals for treatment, monitoring hygiene and teaching independent living skills. Intensively work with clients as they move through the program continuum by assisting residents in obtaining documentation, escorting to designated appointments, securing entitlements, preparation and completion of a comprehensive housing packet (HRA2010E) and make arrangements with housing providers for client's housing interviews. Generate ongoing progress and group notes as well as monthly statistical reports. Participate in clinical team, case conference, and other staff meetings. Assist with program staff coverage as needed. Provide emergency first aid/CPR assistance when needed. Perform additional related duties as assigned.



High School Diploma/GED required; bachelor's degree preferred and masters ideal. Bilingual Spanish preferred, but not required. Excellent communication and writing skills and computer proficiency required. Experience working in residential treatment and with the MICA and homeless populations preferred. Knowledge of HRA and NY/NY application systems a plus. CPR training certification or willingness to take training class in CPR.
