PROGRAM DESCRIPTION:

Located on the Lower East Side, Los Vecinos Apartments is a permanent SRO housing facility with a capacity for 35 formerly homeless men and women suffering from mental illness, substance abuse and HIV/AIDS. The program is funded by the NYC Department of Homeless Services, the NYC Department of Housing Preservation and Development, the NYC Department of Health and Mental Hygiene and the NYC Human Resources Administration's HIV/AIDS Services Administration. In addition to providing permanent housing for its tenants, Los Vecinos offers a variety of services aimed at helping clients stabilize their health, maintain their independence and build a community of support among peers. These services include case management, referrals to medical facilities and assistance managing health conditions, medication monitoring, health education, psychiatric management, entitlement support and money management, recreational activities, assistance with activities of daily living and free meals.

 

HOURS:                                            

Full-time, 37.5 hours per week

Sunday - Thursday; 11:00a - 7:30p

 

DUTIES/RESPONSIBILITIES:    

Develop policies, protocols and forms regarding medications and health management. Administer medication. Make referrals for medical treatment; and follow-up with psychiatric rehabilitation programs, treating physicians, pharmacies, and other treatment collaterals. Serve as the primary medical case manager for assigned clients. Provide individual and group therapy; medical referrals. Facilitate in-service trainings for staff and residents. Attend case conferences as needed. Manage client health needs. Run health-related educational groups. Basic health assessment including blood pressure check, weight, etc. and referrals as appropriate. Participation in treatment team meetings. Assist in organizing and participating in goal development and related activities. Weekly chart documentation and interaction with medical and psychiatric providers. Maintain medication administration record and controlled substance log. Monitor and count medication and medication supply. Related duties as assigned.

 

QUALIFICATIONS:                      

LPN license required. Experience in psychiatric and medical nursing preferred. Excellent organizational skills. Basic computer literacy required.
