PROGRAM DESCRIPTION:

For many years, BRC has provided home-based case management services through its Blended Case Management (BCM) activities. With Medicaid reform, BCM has transitioned into BRC's Care Coordination program. Funded by Medicaid and the NYC Department of Health and Mental Hygiene and Medicaid, BRC Care Coordination offers comprehensive case management services to individuals diagnosed with a serious and persistent mental illness, many with a history of homelessness and/or substance abuse. BRC maintains an ongoing enrollment of over 100 clients in its Care Coordination program.


HOURS:

Full-time, 35 hours per week
Monday - Friday, 9:00am - 5:00pm


DUTIES/RESPONSIBILITIES:

Work with case load (up to 25 clients) to maintain stability within the community. Clients have co-occurring disorders which may include, but not limited to mental illness, substance abuse, and/or serious medical conditions. Will conduct and complete intakes, assessments, and care coordination/treatment plans. Work with other providers to ensure appropriate services are accessed and in place to allow the client to maintain stability within their community. Additional responsibilities include, but not limited to, client escorts, assistance with ADL's, crisis intervention, securing temporary or permanent housing, and other interventions. Documentation, record keeping and progress notes. Attend staff meetings and trainings. Provide specialized care for AOT/ Kendra's Law clients, including all appropriate documentation. Perform Inter RAI Assessments to determine if HARP members are eligible for Home and Community Based Services. On-call responsibilities in response to client crises. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.


QUALIFICATIONS:

Related Master's degree and one ( 1) year of qualifying Experience;

OR

Realted bachelor's degree and( 2) years of qualifying Experience

OR

A credentialed Alcoholism and Substance use Counselor ( CASAC) and two (2) years of qualifying experience;

OR

A bachelors degree or higher in ANY field with either: three (3) years of qualifying experience, or ( 2) years of experience as a Health Home care Manager serving SMI or SED population.
